Planning to Retire

This page links you to the retirement forms and documents to help you through the retirement process. It is recommended that you review the information contained in these pages, make a list of questions you need answered, and gather the necessary documents you will need when you arrive at your counseling session.

Please note that although it is not necessary for you to print and fill out the forms before coming to your counseling session, filling out these forms in advance will allow you more time to ask questions specific to your case. If you are unable to come in for a counseling session you may retire by mail by filling out the necessary forms and mailing them to LACERS. A counselor will contact you by telephone to review your submitted forms.

Forms below are interactive -- you may type information directly on the form. Once completed, print out the form, sign (if applicable), and return it to LACERS via mail, fax, or in person.

Follow the steps below to plan for your retirement.
Step 1. Find out if you qualify for retirement.
 
Step 2. Select a retirement date. See "When is the best time to retire?" from FAQs About the Retirement Process.
 
Step 3. Go to the benefit calculator for an estimate of your retirement allowance.
 
Step 4. Print, fill out, and bring the following forms to your Counseling Session.
Step 5. Review the list of required documents. With your application, please provide copies of the documents outlined in this form, as applicable.
 
Step 6. Read the following informational documents.
Step 7. Find out if you qualify for the LACERS Health Benefit. If so, then follow the 4-Step Guide to Enrolling in LACERS Health Plans.

Step 9. Come to your Retirement Counseling Session with the following documents:

Member Information Sheet
LACERS Medical and Dental Enrollment form
Copy of Medicare Card for you and/or your dependent(s) if applicable
Funeral Expense Allowance Beneficiary form
Designation of Beneficiary form for Unused Contributions and Accrued Payments
