To begin making contributions toward a Larger Annuity or to change
your biweekly contribution amount, please complete and return an Initiation
or Change of Contribution Amount form.
To transfer funds from another retirement fund into a LACERS Larger
Annuity account, please complete a Trustee-to-Trustee
Transfer/Direct Rollover form and forward it to the current holder of your funds. The
current holder of your funds may have other paperwork they will require
you to complete to transfer your funds to LACERS. You will also want
to complete an Initiation
or Change of Contribution Amount form to
select an investment option.
If you
are in the process of retiring and want to purchase a Larger Annuity
from LACERS, please complete an Application
to Purchase Larger Annuity form and forward it to your retirement
counselor. If you plan to fund your Larger Annuity with money from
another retirement fund,
you will also need to complete a Trustee-to-Trustee
Transfer/Direct Rollover form and forward it to the current holder
of your funds.
