Four Steps to Enrolling in LACERS' Health Plans

Please read the LACERS Health and Dental Plans Information Sheet first to answer general questions about your health benefits. The 2008 Benefits Guide is an even more comprehensive description of those benefits.

Step 1: Review LACERS Medical and Dental Plans

Review your LACERS Medical and Dental Benefits online in the 2008 Benefits Guide. This will give you information on eligibility, enrollment, subsidy, and deduction information. Please be sure to read it before making your decisions.

ATTENTION NEW RETIREES - You must enroll within 60 days of retirement or wait until the next open enrollment period, October 1-31.

Step 2: Calculate the Cost of Your Medical and Dental Plans

You must have both the minimum years of Service Credit (10) and the age (55) in order to be eligible for the subsidy. Those who retire no later than September 30, 2003 under the special retirement provisions contained in Section 4.1056.2 of the Administrative Code ("50/30") are also eligible for a subsidy. Step 3: Determine Your Eligibility for Medicare

You and your dependent(s) must enroll in those parts of Medicare for which you and your dependent(s) are eligible in order to receive a LACERS' medical plan premium subsidy and to continue your retiree medical plan coverage.

Your local Social Security Office will help you determine your eligibility and accrued credits. Find your local Social Security Office by calling (800) 772-1213.

If you and/or your dependent(s) have Medicare, you must supply a copy of the Medicare card to LACERS.

Step 4: Submit the Necessary Forms

Once retired, you have 60 days to enroll in a LACERS plan or you must wait for the next Open Enrollment Period, October 1-31. Please take the time to carefully consider what plan(s) best suits your needs and what you can afford. Forms are available in the Forms section of this website and on pages 45 & 46 of the 2008 Benefits Guide, which you received at your retirement counseling meeting and is also available online.

Enrollment Information Checklist

LACERS Medical and Dental Enrollment form.
Enroll in Medicare if you and/or your dependent(s) are age 65 or older.
Additional Senior form necessary if you and/or dependent(s) have Medicare Parts A & B and are enrolling in Kaiser Senior Advantage, Secure Horizons or SCAN.
Choose a medical group if selecting Secure Horizons, SCAN or Blue Cross HMO.
Choose a dental group if selecting SafeGuard.
Copy of Medicare Card for you and/or your dependent(s) if applicable.
Return forms promptly. Must be received by LACERS within 60 days of retirement.
