Adding a Spouse/Domestic Partner
You can add your spouse/domestic partner to your health plan by completing a LACERS' Family Account Change Form within 30 days of their becoming your dependent.
Please return completed and dated LACERS Family Account change form with original signature(s) to:

The Family Account Change form cannot be filed electronically.
